A key component of all projects funded by Garrett Lee Smith Act Grants is evaluation of their impact. Nebraska is participating in a cross-site evaluation and is tracking the impact of all local activities funded by the grant.

School Counselor / Staff Reporting Link:
Part of the evaluation for this project is assessing the impact of QPR, Kognito, and Making Educators Partners in Suicide Prevention (MEP) as suicide prevention trainings in Nebraska schools. When you complete the Identification, Referral and Follow-up (EIRF) form you will be participating in the national evaluation required by all states receiving funding for the prevention of suicide. The questions are based on this form (pdf): Early Identification, Referral and Follow-up (EIRF) Individual Form.


Gatekeeper Reporting Link:
Part of the evaluation for this project is assessing the impact of QPR as a gatekeeper training in Nebraska. When you complete the Identification, Referral and Follow-up (EIRF) form you will be participating in the national evaluation required by all states receiving funding for the prevention of suicide.
